A query made the list when all five of these were true at once:

• Impressions held. Current-period impressions were at least 80% of the comparison period. Demand for the page hasn't dropped.
• Clicks collapsed. Current-period clicks were no more than 60% of the comparison period. People are seeing it, just not clicking it.
• CTR fell hard. Click-through rate dropped by at least 30% in relative terms.
• Ranking stayed put. Average position stayed within 3 places of the comparison period. This filter is the important one. It rules out the simple "you dropped a few spots" explanation.
• Noise filtered. Current-period impressions were at least 50. Tiny query samples are too unreliable to draw conclusions from.

Severity is a 0–100 score built from two parts. 55% is the strength of the suppression signal (50% CTR drop magnitude, 30% impression stability, 20% position stability). The remaining 45% is volume — how many clicks were actually lost — so a query bleeding real traffic outranks one that only shed a click or two. Critical sits at 80 and above, High 60–79, Moderate 40–59. Anything under 40 isn't shown.
